Job summary

Choices Coordinated Care Solutions is seeking an Intercept Specialist to provide intensive home-based interventions and case management for youth and families. The role requires managing 4-6 families, meeting them multiple times weekly, and coordinating services in a strength-based, family-centered approach.

The position emphasizes flexible scheduling, including evenings and weekends, with opportunities to collaborate with a multidisciplinary team and ensure data collection for evaluation.

Qualifications

• Bachelor’s degree in social work or related field required; master’s degree preferred.
• One year of experience with youth and/or families in social services or education.
• Strong communication and writing skills; bilingual Spanish a plus.
• Must have a valid driver’s license and auto insurance.

Responsibilities

• Manage a caseload of 4-6 families within program parameters.
• Meet with families at least 3 times per week and document accurately.
• Provide intensive home-based interventions and coordinate care.
• Offer crisis support and flexible scheduling including evenings/weekends.
• Coordinate with supervisor and team to plan services for youth and families.
