Job summary

Georgia-Families-in-Transition seeks an Intensive Family Intervention Paraprofessional to deliver in-home mental health skill-building and case management for children and adolescents at high risk. Responsibilities include coordination with a multidisciplinary team and crisis intervention when needed.

The role requires a Bachelor’s degree in psychology/social work or related field, 2+ years of experience, and willingness to travel up to an hour to client sites.

Qualifications

• Bachelor’s degree in psychology/social work/human services with at least 2 years of experience.
• Must pass DBHDD background check before providing services.
• Relias, CPR/First Aid, CPI training within 30 days of hire (Relias at no cost; CPR/First Aid and CPI costs not covered).
• Knowledge of Medicaid/CMO compliance preferred.
• Willingness to learn new documentation and case management skills; receptive to feedback.

Responsibilities

• Provide intensive in-home individual/family mental health skill-building and case management.
• Coordinate care with a multi-disciplinary treatment team.
• Provide crisis intervention as needed to support functioning.
• Attend weekly supervision with team lead.
• Meet agency productivity and documentation standards.
• Flexibility to work after-school hours and weekends as needed.
• Travel to client homes and community locations; travel up to 1 hour for sessions.
