Job summary

Health Connect America is seeking a Family Centered Treatment Specialist to provide evidence-based services to children, adolescents and families in home and community settings. You will collaborate with a Treatment Team to develop and implement Service Plans, ensuring fidelity to the FCT model and timely documentation.

The role requires a Bachelor's degree in human services or related field, 2 years' experience, and ideally FCT certification.

Qualifications

• Bachelor's Degree in human services or related field is required.
• Two years’ experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.
• Mississippi: provisionally credentialed as a PCSS with eligibility to be fully credentialed as a CCSS.
• North Carolina: Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis.
• Ohio: LSW or eligibility for provisional licensure preferred.
• Virginia: QMHP or ability to register as a QMHP with the Virginia Board of Counseling.

Responsibilities

• Provide Family Centered Treatment (FCT) services to children, adolescents and families.
• Deliver FCT services primarily in the home and community with a family focus.
• Develop and maintain the Service Plan with the Treatment Team and ensure timely updates.
• Provide 24/7 on-call coverage to a caseload of families and children.
• Complete clinical records and FCT documents per company standards.
• Coordinate discharge plans and refer to community resources as needed.
• Coordinate and oversee initial and ongoing assessment activities.
• Participate in Quality Assurance/Quality Improvement activities.
