CHR Career Opportunities in New Britain, CT is seeking a Family Support Specialist to provide in-home, client-centered services for families while collaborating with a clinical team. This role focuses on youth and family safety, stability, and connection to resources within a community-based program.

The position requires a Bachelor's degree in human services, at least two years in a mental health setting, and a valid driver’s license.

Qualifications

• Bachelor’s degree in human services required.
• At least two (2) years experience in a mental health setting preferred.
• For identified positions, bilingual capacity may be required.
• Valid driver’s license.

Responsabilités

• Provides high quality, client-centered services to children and their families.
• Demonstrates competency in topics of child development, substance use, and trauma-sensitive and culturally competent care.
• Delivers case management, education, referral, and crisis support as indicated.
• Collaborates with other providers to support the needs of the client/family.
• Works in close collaboration with program or agency staff in event of a clinical crisis.
• Assists families in connecting to community and natural supports.
• Participates in treatment and discharge planning.
• Completes all program and agency required documentation, according to program and agency guidelines.
• Documents all client and collateral contacts in client record.
• Supports families in ensuring that youth/families connect and engage in the most appropriate level of care.
