Job summary

Care Planning Institute, Inc. is seeking a part-time field assessor to perform in-home evaluations for elderly clients in the Sioux Falls, SD area. Assessments occur in clients' homes and cover non-medical care needs, with documentation completed on site or afterward.

Assessors must have experience with assessments, be able to travel within a 2-hour radius of Rapid City, and complete assessments within 3–5 days of notification. TB testing may be required, and training will be provided.

Qualifications

• Experience with in-home assessments is required.
• Training will be provided on the job.
• Must be able to travel within Sioux Falls SD region and up to a 2‑hour drive from Rapid City.

Responsibilities

• Conduct in-home assessments with clients in their own homes.
• Ask questions about non-medical care needs (bathing, dressing, grooming, light house cleaning, etc.).
• Fill out health care assessment documents accurately.
• Complete assessments within 3–5 calendar days of notification; travel time paid at $40/hour.
