• Background check and TB skin test required.
• Good communication and observational skills.

Responsibilities

• Perform in-home assessments of elderly clients to determine non-medical care needs (bath, dressing, grooming, light housekeeping).
• Explain needs assessment process and document findings accurately.
• Submit assessment results within 7 calendar days of notification; coordinate with program requirements.
