Job summary

Care Planning Institute, Inc. is seeking a part-time in-home assessor to evaluate elderly clients in the Upper Peninsula, MI area within a 2-hour drive of Marquette. Pay is $40/hour, including paid travel time. Assessments are conducted in clients' homes and take about 30 minutes. Flexible schedule, weekends optional, with background check and TB test required.

Preference given to RN/LVN/LPN license holders, but not required if applicant has home health experience. Training provided; starts soon.

Qualifications

• Assessments conducted in clients' homes
• Ability to travel within 2 hours of Marquette, MI
• Background check required and TB test mandatory
• Flexible scheduling including weekends is acceptable

Responsibilities

• Conduct in-home assessments for elderly clients
• Discuss non-medical care needs (bathing, dressing, grooming, light housekeeping)
• Document findings and coordinate with care planning
• Complete assessments within 7 calendar days of notification
