Herewith is seeking compassionate Care Partners to support patients receiving in-home dialysis, ensuring safe environments and patient wellbeing. You will be matched by skill and personality and work with a clinical team.

Candidates should have at least 1 year of dialysis machine experience, familiarity with home dialysis is a plus, and you must be dependable and empathetic with excellent communication.

Qualifications

• At least 1 year of dialysis patient support experience.
• Familiarity with home dialysis machines preferred.
• Reliable, empathetic and detail oriented.
• Strong interpersonal communication skills.

Responsibilities

• Set up and support dialysis treatments at patient home.
• Provide companionship, reassurance and schedule adherence.
• Monitor safety and assist with light ADLs as needed.
• Maintain communication with nurses and care coordinators.
• Document concerns observed during support hours.
