Job summary

Easterseals Southern California seeks an in-home support professional to assist individuals with disabilities and mental health needs. You will assess, plan, and coordinate services to help participants access community resources and achieve desired outcomes.

Duties include ISP development, family liaison, and ensuring documentation compliance. Requires an Associate’s degree (preference for Human Services), CPR/First Aid, and ability to travel locally for home visits.

Qualifications

• Associate’s degree required, preference for Human Services/related field.
• Excellent communication, oral and written, with diverse audiences.
• Proficiency in MS Office applications (Outlook, Word).

Responsibilities

• Conducts home visits to assess participant needs and progress.
• Develops an Individual Service Plan (ISP) and coordinates services.
• Supports access to community resources and system navigation.
• Liaisons between participants, families, and agencies.
• Maintains documentation and adheres to program guidelines.
• Provides orientation and trainings to associates on service delivery.
