Job summary

BrightSpring Health Services’ Rehab Without Walls Home & Community Program in Tacoma is seeking a Speech-Language Pathologist for PRN, W2 status. You will provide speech, language, cognitive, and swallowing therapy in home and community settings to support functional communication.

Responsibilities include evaluations, treatment planning, progress re-assessment, and collaboration with OT/PT/LCSW teams to optimize patient outcomes, with emphasis on safety and discharge planning.

Qualifications

• Master’s Degree in Speech-Language Pathology
• Valid Speech Language Pathology license in the state(s) of practice
• Current CPR Certification
• At least one year’s work experience as a Speech Language Pathologist
• Knowledge of rehabilitation techniques related to complex neurological injury
• Effective verbal and written communication skills
• Ability to lift 50 pounds; moving, lifting or transferring patients may involve up to 100 pounds following safety procedures
• Fine motor skills, visual acuity, and ability to walk/stand for extended periods
• Health screen or examination may be required prior to assignment and periodically thereafter

Responsibilities

• Perform evaluations and develop treatment plans for patients with speech, language, cognitive and swallowing disorders
• Regularly re‑assess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
• Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members
• Communicate with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
