Qualifications

• Associates required. Bachelor's preferred.
• Three (3) years 3+ years experience in community-based care required.
• Experience with electronic medical record strongly preferred (eCW) preferred.

Responsibilities

• Deliver care to the most complex patients, providing care management and care coordination support.
• Collaborate with external providers to ensure a cohesive medical treatment plan.
• Conduct regular assessments of social determinants of health and engage with patients via visits, telehealth, or outreach.
