Job summary

Kids for the Future is seeking a Community Integration Coach to work with adults in their homes and local communities in Maquoketa, IA. The role focuses on coaching people with complex needs to master daily tasks and live independently in the community.

You'll collaborate with case managers and health professionals, conduct assessments, teach life skills, support health management, and help members navigate community resources while documenting progress in the EHR.

Qualifications

• Two (2) or more years in human services required if no college degree is held.
• Strong interpersonal and written/verbal communication skills.
• Empathy, patience and dedication to helping individuals gain independence.
• Basic computer literacy and knowledge of EHR systems.

Responsibilities

• Collaborate with case managers, therapists, and families to ensure coordinated care.
• Assist in assessments and contribute to Person-Centered Service Plans.
• Teach daily living skills like budgeting, meals, and housekeeping.
• Help clients access community activities and resources.
• Support health management, medications, and scheduling appointments.
• Document progress in EHR and meet agency/state timelines.
• Maintain professional boundaries in diverse living environments.
• Contribute to workplace readiness and community inclusion initiatives.
