Job Summary
The Community Integration Coach works directly with adults in their own homes and local communities who are managing severe mental illness, intellectual disabilities, or brain injuries. In this role, the Community Integration Coach delivers individualized coaching to help members master essential daily tasks, build independent living skills, and connect with their local community. The goal is to empower members to live successfully in the community and avoid institutional placement.
Duties & Responsibilities
- Interdisciplinary Collaboration: Partner closely with supports such as case managers, therapists, employers, medical professionals, and family members to ensure cohesive care. Participate in regular interdisciplinary team (IDT) meetings to review member progress and coordinate interventions.
- Assessments & Service Planning: Assist in conducting assessments for new and existing members. Actively contribute to the development, review, and modification of the individual's Person-Centered Service Plan (PCSP) and goal strategies.
- Daily Living Skill-Building: Teach and coach individuals on essential life skills such as budgeting, meal preparation, personal care, housekeeping, transportation, communication, and problem-solving.
- Community Inclusion & Navigation: Assist individuals in exploring and accessing community activities, including social events, recreational activities, and volunteering to develop social interactions.
- Health & Medication Management: Monitor psychiatric and medical symptoms. Provide cues for self-administration of medications. Teach individuals how to schedule and track doctor appointments and coordinate pharmacy refills.
- Behavioral Support & Crisis De-escalation: Provide immediate assistance and support during crises, connecting individuals to appropriate resources. Help individuals to develop healthy coping skills
- Member Advocacy & Self-Determination: Advocate for the member’s rights, choices, and personal autonomy both within the agency and out in the community. Teach and encourage self-advocacy skills
- Long-Term Job Coaching & Workplace Support (for trained Job Coaches only): Assist members in maintaining their current employment by reinforcing critical workplace habits at home and on the job site. Teach skills related to workplace communication, time management, navigating social dynamics with coworkers, and managing job-related stress. Partner with the employer and the member's support team to implement long-term accommodations that ensure ongoing job retention.
- Clinical EHR Management & Timely Documentation: Maintain a secure, accurate, and up-to-date Electronic Health Record (EHR) for each assigned member. Document comprehensive progress notes, ensuring all entries are finalized within agency-mandated and state Medicaid timelines. Ensure all clinical documentation clearly measures progress toward service goals. Report all critical incidents and complete required documentation per state and agency guidelines.
- Productivity & Billable Hours Compliance: Meet or exceed the agency’s established productivity requirements by efficiently managing your schedule and caseload. Consistently deliver the target number of billable service hours as outlined by management, ensuring all scheduled appointments are attended, documented, and accurately tracked to support organization stability.
- Professional Development & Evidence-Based Training: Stay updated with best practices in community integration, mental health, and disability support through ongoing training. Complete all mandatory agency and state training. Attends department team meetings as scheduled, also.

Job Requirements
- Experience:Two (2) or more years of experience in the human services field required if no college degree is held.Experience with HCBS programs preferred.
- Knowledge/Skills: Strong interpersonal and communication skills – including written and verbal.Patience, empathy, and a passion for supporting individuals to achieve independence.Ability to work with diverse populations and cultural backgrounds.Problem-solving abilities and creative thinking.Familiarity with local community resources.Ability to work independently and as part of a team.Basic computer literacy (Microsoft Office, electronic health record).
- Licenses/Certifications:Valid Driver’s License, access to reliable personal vehicle to work on community goals with clients. Must meet eligibility to be an agency driver
Community & Home Environments
Services are provided directly within the private residences of members. These environments vary greatly and may include homes that are unkempt or cluttered. Employees may be routinely exposed to secondhand cigarette smoke, household pets, and various allergens. Must be able to navigate stairs. Candidates must be comfortable adapting to and maintaining professional boundaries within diverse living conditions.
