Job summary

All Care Companions LLC in Eagan, MN is seeking a compassionate In Home and Community Care Support Individual to join our team. You will provide personalized care at clients' homes and in the community, making a meaningful difference in their lives.

Responsibilities include direct personal care and ADLs, accompanying clients to appointments and activities, monitoring health and progress, and implementing individualized care plans.

Qualifications

• Genuine compassion, patience, and dedication to supporting individuals with diverse needs.
• Strong communication and interpersonal skills.
• Ability to pass a background check as required by state regulations.
• Valid driver’s license and reliable transportation.
• Ability to work independently and as part of a collaborative care team.
• Prior experience in home care, PCA, direct support, or related caregiving preferred.

Responsibilities

• Provide direct, hands-on personal care and daily living assistance to clients in home and community settings
• Support clients with activities of daily living (ADLs) including bathing, grooming, dressing, and meal preparation
• Accompany and assist clients with community outings, appointments, and social activities
• Monitor and document client health, behavior, and progress, reporting changes to supervisors
• Implement individualized care plans tailored to each client’s needs and goals
• Foster a safe, supportive, and dignified environment for all clients
• Build trusting relationships with clients, families, and care team members
