Job summary

Qualifications

• Assists with activities of daily living (ADLs) including dressing, grooming, toileting, and safe transfers.
• Performs meal preparation, light housekeeping, and errands while maintaining a clean, safe home environment.
• Communicates effectively to build trust, provide emotional support, and collaborate with families and care teams.

Responsibilities

• Provide direct home-based support to clients, following individualized care plans.
• Assist with personal hygiene, mobility, grooming, dressing, and daily routines.
