Job summary

All Care Companions LLC in Eagan, MN is seeking a compassionate In Home and Community Care Support Individual to provide personalized care to clients in home and community settings. Strong communication and interpersonal skills.
Genuine compassion, patience, and dedication to supporting individuals with diverse needs.
Prior experience in home care, PCA, direct support, or related caregiving role preferred.

Responsibilities

Provide direct, hands-on personal care and daily living assistance to clients in home and community settings.
Support clients with activities of daily living (ADLs) including bathing, grooming, dressing, and meal preparation.
Accompany and assist clients with community outings, appointments, and social activities.
Monitor and document client health, behavior, and progress, reporting changes to supervisors.
Implement individualized care plans tailored to each client's unique needs and goals.
Foster a safe, respectful environment and build trusting relationships with clients, families, and care team.
