Signify Health, part of CVS Health, seeks a Full Time Clinician (NP/PA) to perform in-home health evaluations. Visits last ~45 minutes, with comprehensive history, vitals, and optional diagnostics. Travel to licensed states may be required; virtual visits possible.

Board-certified clinicians provide care without changing current regimens, using an iPad-based workflow. Eligible candidates must hold an active license and be prepared to travel nationally, with benefits including malpractice

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
