Qualifications

• Current certification/registration with the Licensing Board or Department in state of practice.
• CPR certification preferred.
• Bi-lingual English/Spanish a plus.
• Must meet transportation, language, and cultural requirements for branch.

Responsibilities

• Implements day-to-day ADL and IADL support per physician plan and client care.
• Assists with personal care like baths, grooming, dressing per care plan.
• Assists with elimination, transfers, ambulation as directed.
• Supports therapy exercises under supervision of a Registered Therapist.
• Assists with meals and basic household tasks per care plan.
• Provides companionship and escorts to appointments.
• Reminds client to self-administer medications (does not administer).
• Documents shift details accurately within time parameters.
• Maintains safety, follows Universal Precautions and HIPAA.
• Reports changes in client status promptly.
• Maintains skills through staff development and training.
• Follows all agency policies; performs other duties as assigned.
