Qualifications

Responsibilities

• Provide in-home para-professional services under RN direction.
• Assist patients with personal care activities and daily living tasks.
• Record all information accurately and maintain patient charts.
• Assist with transfers, mobility, and range-of-motion exercises.
• Perform light housekeeping tasks and meal assistance as needed.
• Support medication administration and ostomy care as required.
• Adhere to safety protocols and organizational standards.
