Job summary

Fairview Hospital / Cleveland Clinic is seeking a dedicated direct-care aide to provide one-on-one support to adults with intellectual and developmental disabilities in community-based and home settings. Key duties include assisting with daily activities, documenting progress, managing medications under guidance, and upholding health and safety standards.

Deliver care per individual plans while maintaining confidentiality and reporting incidents.

Qualifications

• High school diploma or equivalent preferred.
• Six months of human services experience preferred.
• Must be 18+ with a valid driver’s license, computer skills, and ability to perform physical tasks.
• Completion of mandated training and background checks are required.

Job description

Key Responsibilities: Assist with daily activities such as personal care, housekeeping, transportation, socialization, and community participation. Document progress, support medication management, and ensure health and safety standards are met.

Work Environment & Physical Demands: Work primarily in community and home settings, with physical requirements including exerting up to 100 pounds of force. Maintain a safe environment and follow safety protocols.

Additional Information: No supervisory responsibilities. Must be reliable, punctual, team-oriented, and committed to providing quality care to promote independence and well-being of individuals served.
