Job summary

CARIN Alliance in Prescott, AZ seeks a compassionate caregiver to provide one-on-one support to adults living independently or with caregivers, helping them stay healthy and engage with the community in home-based settings.

You will implement individualized plans, assist with daily activities, health management, meals, and transportation, while maintaining records and upholding safety and rights.

Qualifications

• High school diploma or equivalent preferred.
• Six months of human services experience preferred.
• Must be 18+ with a valid driver''s license and basic computer knowledge.
• Able to perform physical tasks up to 100 pounds.

Responsibilities

• Provide one-on-one support to promote independence and community participation.
• Assist with daily activities including socialization, health management, medication, personal care, and transportation.
• Maintain accurate records and report incidents or concerns appropriately.
• Support medical appointments and health monitoring.
• Prepare meals per dietary needs and ensure safety during health-related activities.
• Attend training and work scheduled hours; ensure safety and compliance.

Job description

Role Overview:

Provides one-on-one support to adults living independently or with caregivers, promoting their independence and community participation. Works within home-based settings under the IHSS and HCBS programs, ensuring individuals' health and safety while assisting with daily activities.

Key Responsibilities:

Implements individualized support plans, assisting with socialization, health management, medication, personal care, housekeeping, transportation, and community engagement. Maintains accurate records, respects individuals' rights, and reports any incidents or concerns appropriately.

Health Care Duties:

Supports medical appointments, administers or facilitates medication, monitors health, and handles medical supplies. Prepares meals per dietary needs and ensures safety during health-related activities.

Qualifications & Skills:

High school diploma or equivalent preferred; six months of human services experience preferred. Must be 18+, possess a valid driver’s license, and have basic computer knowledge. Ability to perform physical tasks, including exerting up to 100 pounds of force.

Additional Expectations:

Attends training, works scheduled hours, maintains a safe environment, and demonstrates teamwork and professionalism. May transport individuals, perform minor household repairs, and participate in safety drills.

Supervisory & Environment Info:

No supervisory responsibilities. Works mainly at program locations and in the community, with an emphasis on safety and compliance with policies and regulations.
