Guardian Angel Senior Services, Inc. is seeking staff to provide Individual Support to Massachusetts Waiver Contracted Individuals, helping them maximize independent functioning in self-care, physical and emotional growth, socialization and communication.

Responsibilities include collaborating with an Interdisciplinary team, developing care plan information, and documenting visits, while maintaining confidentiality and complying with state and agency policies.

Qualifications

• BA in a Human Service-related field preferred.
• Ability to research and identify other sources of information, advice, guidance and support.
• Strong IT skills, including managing websites and knowledge of all forms of social/media communications.
• Good listening and communication skills.
• Be able to travel around the borough independently, preferably by car.
• Be flexible around working hours and prepared to work some evenings or weekends as required.
• Familiar with the Greater Boston area community support system.
• Committed to the rights of seniors and disabled people to live dignified lives.
• Prior home care experience helpful.
• Willing to travel throughout Western MA areas.

Responsibilities

• Empower individuals to contribute to decisions about education, health and care provision to achieve better outcomes.
• Assist in developing information concerning care plans.
• Work with and encourage partnership between individuals and the community.
• Collaborate with the Interdisciplinary team involved in an individual's care.
• Provide resources to help maintain safety and independence in the community.
• Assist with training and supervision of caregivers and PCAs.
• Undertake home visits and document activity.
• Carry out clerical and administrative duties as required.
• Maintain client confidentiality and comply with policies.
