Job summary

St. Luke’s Health System is seeking a Home Care RN to deliver in-home nursing care to patients in Boise and surrounding areas. The role supports Home Health and Hospital-at-Home services, focusing on patients with new diagnoses, new medications, respiratory needs, and wound care.

You will educate patients and families and coordinate care plans during visits. The position offers a full-time weekend schedule, on-call compensation, and additional pay differentials.

Qualifications

• Education: Nursing Degree (ASN or BSN)
• Experience: At least 1 year RN experience; Home Health or Acute Care experience preferred
• Licenses/Certifications: Current RN license in Idaho and BLS certification
• Other Requirements: Valid driver’s license and proof of auto insurance

Responsibilities

• Provide comprehensive in-home care to patients with new diagnoses, new medications, respiratory needs, wound care, and those discharged from the ER.
• Monitor progress on care plans during visits and educate patients and families on disease processes and self-management.
• Coordinate resources and discharge planning to ensure smooth transitions back to home or to another level of care.
