St. Luke’s Home Health Boise Team is hiring a Home Care RN to provide in-home care under Home Health and Hospital-at-Home services.

You will assess patients, manage care plans, educate on disease processes, and coordinate discharge resources to support families. This full-time role offers a weekend schedule, on-call rotation, hourly pay with drive time and mileage reimbursement, plus sign-on and relocation assistance.

Qualifications

• Education: Nursing Degree (ASN or BSN)
• Experience: At least 1 year RN; Home Health or Acute Care experience preferred
• Licenses/Certifications: Current RN license in Idaho and BLS certification
• Other Requirements: Valid driver’s license and proof of auto insurance

Responsibilities

• Provide in-home RN care and manage patient care plans
• Educate patients and families on disease processes and self-care
• Coordinate discharge resources and onward care as needed
