Job summary

Mercy provides day-to-day patient care in the home setting, including case management and total care for multiple patients with varying acuity. The per diem role supports admissions, post-hospitalization assessments, discharge planning, and patient teaching, with documentation in Epic.

Requires RN licensure, graduation from an accredited nursing program, and CPR; driving is required with a valid license. Three years of nursing experience, including hospital experience, is preferred.

Qualifications

• Graduation from accredited school of nursing.
• RN license in applicable state.
• Valid driver's license with clean record.
• Three years nursing experience, with minimum one year in hospital setting; home health preferred.
• Per diem requires minimum one year Home Health experience.
• CPR biannually.

Responsibilities

• Provide day-to-day patient care in the home setting and case management for multiple patients.
• Admissions to home care services and post-hospitalization assessments; recertifications, discharges, patient teaching.
• Administer IV therapy and provide dressing changes; ensure safety and quality care in compliance with policy.
