The position requires an active RN license in the practice state, CPR certification, and a minimum of three years nursing experience with at least one year in hospital settings; home health experience is preferred.

Qualifications

• Graduation from accredited school of nursing.
• RN license in applicable state of practice with good record.

Responsibilities

• Provide day-to-day patient care in the home setting.
• Case management and total care of multiple patients with varying acuity.
• Documentation in Epic per department policy.
• Per Diem visits as needed following Mercy policies.
