Job summary

Qualicare Home Care Services seeks a compassionate, dependable, and clinically skilled part-time Registered Nurse to serve clients in Paramus, Ridgewood, Fort Lee, Hackensack, and surrounding Bergen County communities.

The RN will assess client needs, develop and update Plans of Care, provide skilled nursing services, and supervise and support Certified Home Health Aides while collaborating with physicians and other professionals to ensure coordinated care.

Qualifications

• Active New Jersey RN license in good standing.
• Current CPR certification.
• Strong clinical assessment and critical-thinking skills.
• Experience in home health, geriatric, rehab, or community nursing preferred.
• Experience supervising Certified Home Health Aides preferred.
• Knowledge of medication management, infection control, and care planning.
• Ability to travel throughout Bergen County and have reliable transportation.
• Proficient with scheduling and clinical documentation software.
• Excellent communication and organization with strong customer-service orientation.

Responsibilities

• Conduct initial and ongoing in-home nursing assessments
• Develop, review, and update individualized Plans of Care
• Provide direct nursing care based on client needs
• Monitor clients and identify changes in condition
• Communicate significant findings to families, physicians, and the care team
• Complete accurate and timely clinical documentation
• Coordinate services with healthcare professionals and caregivers
• Conduct supervisory visits and evaluate care provided by home health aides
• Provide education and clinical guidance to caregivers and families
• Follow infection-control, safety, HIPAA, and agency requirements
• Report incidents, concerns, and changes in client condition
