Job summary

Right at Home is seeking a Licensed Physical Therapy Assistant to provide in-home care with a focus on mobility, gait training, and post-surgical rehab. The role offers a flexible PRN schedule and the opportunity to impact clients in their own homes.

Ideal candidates have home care experience, strong communication, and the ability to educate families while coordinating with other care professionals. A company vehicle is provided for visits.

Qualifications

• Must be Licensed Physical Therapy Assistant with Home Care experience.
• Experience with mobility and gait training in home care.
• Experience with post-surgical rehabilitation programs.
• Ability to implement fall prevention strategies.
• Educate clients and families; coordinate care effectively.
• Strong communication and documentation skills.

Responsibilities

• Provide in-home physical therapy services to clients.
• Develop and implement functional mobility and gait training programs.
• Educate clients and families on care plans and safety.
• Coordinate with care teams to ensure seamless service.
• Document patient progress and treatment outcomes.
• Travel may be required up to one hour one way.
