Qualifications

• Experience in home health or elder care settings.
• Ability to provide personal care and companionship with dignity and respect.
• Knowledge of HIPAA and patient privacy regulations.
• Ability to monitor health changes and report to healthcare professionals.
• Comfort with assisting memory care patients and dementia care needs.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming.
• Monitor vital signs and report changes in health or behavior.
• Prepare meals and assist with dietary needs.
• Maintain a clean, safe home environment and perform light housekeeping.
• Administer medications as prescribed and follow care plans.
• Engage clients in social activities to promote mental well-being.
• Use equipment (e.g., Hoyer lifts) for safe transfers.
• Document care provided and communicate with healthcare professionals as needed.
