Job summary

Qualifications

• High School Diploma or GED required.
• 2+ years in care management preferred.
• Valid Oregon driver license with auto insurance.
• Strong written and verbal communication with diverse populations.

Responsibilities

• Coordinate and monitor services, including tracking of client activities in relation to the client's care plan.
• Oversee the maintenance of comprehensive client files and other documentation of all client encounters.
• Perform home visits as needed.
• Receive and process telephone calls and electronic communications.
• Assist in development of home service plans.
• Ensure Oregon Administrative Rules are followed.
