Job summary

Avid Health at Home in Fayetteville, NC is hiring a Care Coordinator to manage day-to-day scheduling for qualified field staff and coordinate with clients. The role emphasizes maintaining client services, intake of new clients, and accurate EMR updates to ensure ADL/IADL needs are met by the multidisciplinary team.

The position is office-based with some community events, covering Cumberland, Columbus, and Richmond counties.

Qualifications

• Education: College degree preferred or equivalent work experience.
• Licensure/Certification: CNA license preferred.
• Travel: Valid driver’s license and reliable vehicle required.
• Work Experience: Minimum of two years’ related experience.

Responsibilities

• Coordinate day-to-day scheduling of field employees with clients.
• Maintain client services and intake of new clients to the agency.
• Update client care information in EMR to ensure ADL/IADL needs are met.
• Ensure EVV compliance and timely payroll processing.
• Manage timesheets and payroll for branch staff.
• Provide training and mentoring as needed.
