Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to serve Beaumont, TX, in-home. You will perform psychiatric evaluations, develop plans of care, and coordinate with an interdisciplinary team to support homebound patients.

The role requires an RN license, 1–2 years in relevant settings, and a reliable vehicle. Expect ongoing documentation, QA, and collaboration with physicians and families.

Qualifications

• Must have 1-2 years of nursing experience, preferably in acute psychiatric settings.
• Current state RN license is required.
• CPR certification required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home psychiatric, medical, and psychosocial assessments.
• Develop, implement, and update individualized plans of care.
• Provide skilled behavioral health nursing interventions.
• Coordinate care with physicians, families, and referral agencies.
• Ensure medication profiles are accurate and current at all times.
• Complete admissions, resumptions of care, and discharges per standards.
• Maintain EMR documentation and ensure timely charting.
