Integrated Services for Behavioral Health in Pickaway, OH, is seeking a Care Coordinator to connect children, youth, and families with needed services in home, school, and community settings.

The role emphasizes engaging families, coordinating across systems, and ensuring voice and choice in all coordinated care plans. A valid driver's license and wraparound training are preferred. Competitive hourly pay with benefits is offered.

Formación

• Three years with a high school diploma or equivalent; or two years with an associate degree or bachelor’s degree; or one year with a master’s degree or higher.
• Hi-Fidelity Wraparound certification preferred (provided at time of employment).
• Valid Driver's License required.

Responsabilidades

• Identify care coordination needs and connect families to services.
• Define cultural factors influencing strengths and family dynamics.
• Coordinate family-based services in home, school, and community.
• Ensure services meet safety and treatment needs of child and family.
• Engage families with child and family teams to support care plans.
• Develop partnerships with community resources to meet diverse needs.
• Maintain documentation and participate in program planning meetings.
• Remain current with required training.
