Job summary

Integrated Services for Behavioral Health (ISBH) in Pickaway, OH seeks a Care Coordinator to link families with essential services and coordinate care across home, school, and community. You will engage children, youth, and families, identify supports, and help ensure voice and choice in planning.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector human services.
• Three years with a high school diploma or equivalent; two years with an associate or bachelor's; one year with a master's or higher.
• Knowledge/experience with Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
• Strong organizational and communication skills; ability to work across multiple systems.

Responsibilities

• Identify care coordination needs with families to ensure best outcomes for children, youth, and families.
• Coordinate family-based services in home, school, and community.
• Engage families and build partnerships with community resources to support care planning.
• Maintain documentation and participate in program evaluation and team meetings.
• Stay current with required trainings and certifications.
