Job summary

Qualifications

• Graduate from an accredited school of nursing.
• Active, unrestricted Massachusetts RN license.
• Certification in Case Management strongly desired.
• Must have valid driver’s license and reliable transportation.

Responsibilities

• Conducts home visits for onboarding and regulatory assessments.
• Performs HRAs, PCA assessments, and submits MDS assessments.
• Provides education on NaviCare case management program.
• Collaborates with care team and conducts telephonic assessments when appropriate.
• Supports LTSS evaluations and contributes to UM and quality initiatives.
