Job summary

Credent Care LLC is seeking a dedicated ARMHS-Mental Health Practitioner to provide in-home rehabilitative services to adults with mental illness. You will develop treatment goals, document progress, and coordinate care with the treatment team to support independence and community living.

Qualified candidates have DHS credentials for Mental Health Practitioner, 2,000+ hours supervised experience, and strong documentation skills.

Qualifications

• Has completed 30 semester hours or 45 quarter hours in behavioral sciences or related fields and at least 2,000 hours supervised experience in delivering services to adults or children with mental illness or related conditions.
• Has at least 4,000 hours of supervised work experience delivering services to adults or children with mental illness or related conditions.
• Experience with mental health services documentation and progress notes within required timelines.
• Knowledge of DHS regulations and ability to work independently with flexibility.

Responsibilities

• Develop, implement, evaluate, and revise treatment goals
• Provide ongoing in-home or community-based services
• Travel within Central Minnesota region as needed
• Document services and complete progress notes within 24-48 hours
• Coordinate services with other health care providers and interpreters
