Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active and unrestricted licensure in the practicing state
• Active and unrestricted driver's license
• Access to reliable transportation for home visits
• Ability to navigate varied home environments during assessments
• If independent practice is allowed where you practice, obtain required approvals within specified timeframes

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other health plan members
• Identify and address care gaps
• Educate patients on disease management, medications, and preventive care
• Address social determinants of health and connect members with resources
• Collaborate with an interdisciplinary care team in a field-based role
