Job summary

Flourish Health in Bridgeport, OH or Wheeling, WV is seeking an Adolescent Mental Health Guide to support teens and young adults with serious mental illness through in-person and app-based engagement. You will work with an integrated clinical team, travel to homes, and help families access care and resources.

The role emphasizes relationship-building, adaptability, and collaboration within a multidisciplinary team, with a focus on trauma-responsive care and patient-centered engagement.

Qualifications

• Must be 18 years or older.
• Open to individuals with Associate's, Bachelor's, or Master's degrees or 5+ years of experience.
• Experience working with_adolescents, young adults, and families is preferred.
• Bilingual Spanish and English abilities are a plus.

Responsibilities

• Travel to patients' homes to support engagement with the virtual clinic (approx. 2–3 days in Ohio or neighboring states).
• Establish and maintain trusted relationships with Flourish Health patients and families using a strengths-based, trauma-responsive approach.
• Engage patients via app-based messaging and phone calls to meet needs.
• Coordinate care with clinicians and advocates to schedule and communicate appointments.
• Support adherence to individualized care/engagement plans and connect patients to needed resources.
• Maintain a working vehicle for in-home visits and be adaptable to evolving workflows.
