Job summary

Qualifications

Responsibilities

• Provides direct services to clients while maintaining confidentiality.
• Creates an atmosphere of cooperation with clients, families and associates to ensure optimal care.
• Follows behavior management and nursing plans and other directives with accuracy.
• Exercises good judgment in decision-making for client’s best interest.
• Completes required documentation (progress notes, SALs, etc.) accurately and timely.
• Maintains all documentation in compliance with regulations and deadlines.
• Meets established productivity standards.
• Participates in IDT planning and community services teams.
• Transports clients per treatment/service plans and schedules.
