Community Access is a leading provider in New York City, delivering Integrated Mobile Treatment (IMT). The IMT Behavioral Health Specialist plays a key role within a multi-disciplinary team, engaging participants through assessment and therapeutic planning to support wellness, housing stability, and recovery goals.

Applicants should have a Master's degree in a related field, NY fingerprint clearance, and the ability to work flexibly with diverse populations.

Qualifications

• Master's Degree in Social Work or related field; LMSW preferred.
• Minimum of two years' experience with individuals with mental health conditions and co-occurring substance use.
• Available to work a flexible schedule including mornings, evenings and weekends.
• Ability to work in a multi-disciplinary team.
• Must be fingerprinted and cleared by the New York State Justice Center.
• Proficient in Windows and MS Office; comfortable with new electronic systems.

Responsabilités

• Engage participants through individualized assessment and therapeutic planning.
• Use evidence-based practices to support wellness and recovery.
• Apply harm reduction strategies including motivational interviewing.
• Collaborate with a multi-disciplinary team to deliver integrated services.
• Complete required assessments and maintain documentation as needed.
