Job summary

The Arc Fresno/Madera Counties is seeking a dedicated Case Manager of Vocational Services in Fresno, CA. The role focuses on case management, counseling, and implementing Individual Service Plans to promote independent functioning.

Responsibilities include coordinating services, ensuring accurate documentation, and collaborating with referral sources and program staff to meet client goals.

Qualifications

• Associate's degree from an accredited college or university; and one to two years related experience and/or training; or equivalent combination of education and experience.

Responsibilities

• Provide case management services, counseling, intervention and implementation of Individual Service Plans (ISP).
• Coordinate services for assigned caseloads and ensure documentation is complete and accurate.
• Train staff on proper documentation and assist individuals with using ADP software to clock in and manage time and pay documents.
• Monitor billing and ensure services are delivered within approved hours and funding sources.
• Engage with individuals and staff regularly and participate in interdisciplinary team meetings.
