Job summary

Foundations for Home and Community in Maryland is seeking a Therapeutic Foster Care Social Worker to provide comprehensive case management for youth, foster families, and birth families within our therapeutic foster care program.

You will collaborate with schools, community partners, and mental health professionals to ensure safety, permanency, and positive outcomes, including home visits, court involvement, and coordinated treatment plans.

Qualifications

• Bachelor's degree in Social Work required; Master's preferred
• Current LBSW or LMSW licensure required
• Minimum of two years of direct clinical experience with children, adolescents, adults, and/or families
• Previous therapeutic foster care or child welfare and/or case management experience preferred
• Knowledge of community resources, crisis intervention, treatment planning, and child development
• Strong organizational, assessment, communication, and relationship-building skills
• Ability to work independently in community settings, participate in an on-call rotation, and maintain electronic documentation
• Willingness to attend home visits, court hearings, and treatment team meetings

Responsibilities

• Provide comprehensive case management and family-centered services for children in therapeutic foster care
• Conduct assessments, social histories, develop individualized treatment plans, and monitor client progress through home visits and meetings
• Coordinate treatment services, facilitate Family Treatment Team Meetings, and advocate for clients with community providers
• Maintain timely documentation and attend court hearings and related meetings
• Participate in community-based services, including evening visits and on-call rotation
