Job summary

REM Community Services, a part of Sevita, seeks a qualified Social Worker to evaluate and treat clients in home and community settings. You will develop treatment plans, document progress, and provide counseling to clients and families, while collaborating with the care team to set achievable goals.

Requirements include a Master’s in Social Work, NASW membership with related rehabilitation experience (brain injury and spinal cord injury focus), and MI social worker certification.

Qualifications

• Master’s Degree in Social Work from an accredited school of social work.
• Membership in NASW and related clinical experience in a rehabilitation setting with focus on traumatic brain injury and spinal cord injury.
• In Michigan, certified by the State of Michigan as a social worker.

Responsibilities

• Evaluate and treat clients admitted for treatment.
• Develop and implement treatment plans based on physician orders.
• Prepare timely written documentation of client progress including evaluations and discharge summaries.
• Provide one-to-one counseling with clients and/or family members.
• Support brain injury education to clients and caregivers.
• Attend team conferences and communicate client progress to the team.
• Submit billing information in a timely manner.
• Communicate effectively with internal case managers.
• Travel as needed.
