Job summary

REM Community Services, part of Sevita, delivers home and community-based health care for adults, children, and families across the United States. Social Workers evaluate and treat clients in home and community settings, following physician orders and treatment plans.

The role requires a Master’s in Social Work, NASW membership with related rehab experience (TBI/SCI), and a Michigan driver’s license for MI staff. Travel is required as needed.

Qualifications

• Master’s Degree in Social Work from an accredited school.
• NASW membership and rehabilitation-focused clinical experience.
• Valid Michigan driver’s license (for MI-based staff).

Responsibilities

• Evaluate clients and develop treatment plans per physician orders.
• Provide one-to-one counseling with clients and/or family.
• Prepare progress notes, discharge summaries and related reports.
• Attend team conferences and report on client progress.
• Complete billing information promptly.
• Communicate effectively with internal case managers.
