REM Community Services, a part of Sevita, seeks a qualified Social Worker to evaluate and treat clients in home and community settings. You will develop treatment plans, document progress, and provide counseling to clients and families, while collaborating with the care team to set achievable goals.
Requirements include a Master’s in Social Work, NASW membership with related rehabilitation experience (brain injury and spinal cord injury focus), and MI social worker certification.

Social Workers are responsible for the evaluation and treatment of clients admitted for treatment. The Social Worker adheres to specific scope of service for activities related to the evaluation and treatment of clients in the home and community.
