Job summary

Intermountain Centers in Tucson, AZ seeks a SUD Justice, Hospital & Community Liaison to coordinate care across criminal justice resources, hospitals, and community settings. You will work under the SUD Project Manager to ensure timely re-entry, adherence to appointments, and health guidance, while supporting care coordination and advocacy.

The role requires a Bachelor’s degree in a behavioral health field or BHT with two years of behavioral health experience, and a valid AZ driver’s license.

Qualifications

• Bachelor’s degree in a behavioral health related field or BHT level with two years of behavioral health experience.

Responsibilities

• Coordinate with criminal justice resources, hospitals and other community settings.
• Coordinate outpatient and peer support services to avoid gaps in care and ensure continuity.
• Facilitate psychoeducational groups to decrease recidivism.
• Complete documentation for behavioral health records per agency and grant requirements.
• Participate in assessment, evaluation, planning of patient care and complete SDOH assessment.
• Provide case management, health promotions, skills training, and assessment.
• Maintain minimum documented client contact hours weekly.
• Collaborate with referral sources to continue referrals.
• Provide client advocacy and education to ensure service delivery.
• Perform other duties as assigned related to agency growth.
