TURN in Fresno is seeking a Case Manager to support SUD programs. This regular full-time role focuses on access to medical, educational, social, vocational, and housing resources, coordinating referrals, and advocating for clients.

Applicants should have a master's degree or a bachelor's with four years in social services; CA driver’s license; experience with mental health or substance use services; and proficiency with MS Office and EHR systems.

Qualifications

• Master's degree or Bachelor's with four years in social services.
• SUD CM board certification and registration preferred.
• Housing role requires AA in social services or 5 years’ experience.

Responsibilities

• Provide case management with crisis intervention and discharge planning.
• Document accurately, meet billable requirements where applicable.
• Coordinate placements, transport, and field-based services.
• Maintain client confidentiality and participate in program meetings.
• Other duties as assigned.
