Job description

Jail Liaison is primarily responsible for the coordination of care and Community Re-Entry. This position ensures effective and efficient delivery of service(s) to members and their families, as required. General supervision is received from the Community Re-Entry Manager. This includes all documentation of services and outcomes provided.

Primary Duties/ Responsibilities:
  • Complete community re-entry visits for the Adult Detention Center.
  • Complete jail visits, working in collaboration with community agencies.
  • Complete assessments and SMI determinations.
  • Prepare weekly and monthly reports on program status and accomplishments; submit reports to Arizona Complete Health as required.
  • Coordinate services provided with other organizations and social service agencies and refer members to other community resources and organizations as needed.
  • Health Services; coordinate access to entitlements through Health-e Arizona.
  • Respond to requests from the community to engage people who may need behavioral health services.
  • Provide PCPs, Emergency Departments and local stakeholders with information regarding entitlements, behavioral health services and benefits of behavioral health treatment.
  • Coordinate access to behavioral health services for individuals such as assisting in the agency intake process and provide ongoing encouragement.
  • Establish and maintain positive, productive working relationships with all possible referral sources, traditional and non-traditional, including:
    • Human service programs
    • Shelter programs
    • First Responders and other local officials
    • Courts
    • Business community
    • Medical community
    • Government agencies
    • Child Protective Services
    • Schools and other educational institutions
  • Use Motivational Interviewing techniques to engage community members into care.
  • Demonstrate continuous efforts to improve operations, decrease turnaround times, streamline work processes, and work cooperatively and jointly to provide quality seamless customer service.
  • Manage highly sensitive and confidential information.
  • Perform special assignments, projects, and other duties as assigned, consistent with the scope and purpose of the position.
Minimum Qualifications:
  • High school Diploma or GED required.
  • Bachelor’s degree in behavioral health field or four years related experience or training; or equivalent combination of education and experience in behavioral health care to qualify as a behavioral health technician is preferred.

  • Employment is contingent upon the successful completion of a Level One Fingerprint Clearence Card.
  • Reliable transportation, clean driving record, Arizona driver’s license, and current personal automobile insurance required.
