Job summary

Phoenix Rescue Mission, a faith-based nonprofit in Phoenix, AZ, seeks an Outreach/Case Manager to help connect homeless individuals with services and support. You will develop personalized plans and coordinate with community partners while maintaining client confidentiality and data accuracy.

The role emphasizes compassionate engagement, organizational skills, and a credible Christian witness within a caring team. Eligibility includes a Bachelor’s in a related field or 4+ years’ experience.

Qualifications

• Bachelor’s degree in Social Work, Sociology, Psychology, Counseling, or 4+ years in case management.
• Certified in Peer Support is preferred.
• Knowledge of street homelessness and resources to exit homelessness.
• Valid Arizona driver’s license with clean driving record (2+ years).
• Ability to stay calm in stressful situations and maintain confidentiality.
• Excellent communication skills; proficient in MS Office.
• Solid Christian witness and lifestyle.

Responsibilities

• Connect with and motivate people living on the streets to strive toward self-sufficiency.
• Assist homeless people in navigating social service systems to identify resources.
• Set goals and develop individualized case plans with each outreach prospect.
• Establish and maintain relationships with community stakeholders and attend outreach events.
• Ensure client information is entered timely and accurately in the PRM database.
• Lead or participate in devotions, prayers, or other organization activities.
