Job summary

YAI - Seeing beyond disability is seeking a START Coordinator to manage a caseload of 20–25 individuals and provide systemic consultation across settings. The role uses assessments, ecomaps, and outreach to guide services and coordinate with clinical leadership.

Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or related field.
• Minimum two years of experience with I/DD or behavioral health needs; or a satisfactory education/training combination.
• START Coordinator Certification within 12 months of appointment.
• Ability to provide on-call crisis coverage as designated by leadership.
• Ability to travel regularly across regions and states as needed.
• Excellent written and verbal communication; independent and organized.

Responsibilities

• Maintains an active caseload of up to 20–25 individuals referred for START Services.
• Provides systemic consultation based on assessment and outreach findings.
• Conducts regular outreach visits to individuals’ home, day, or vocational settings, in-person or telehealth.
• Assists with referrals, enrollments, intake assessments, and coordination with clinical teams.
• Develops Comprehensive Service Evaluations, Crisis Prevention Plans and intervention plans.
• Participates in meetings with START leadership, clinical team, and Therapeutic Coaching program.
• Provides on-call support on a rotational basis and ensures timely documentation in systems.
